Pay Range: $24.62 - $27.12 Hourly; Based on experience
RESPONSIBILITIES:
- Serve as the primary liaison between families/guardians and the interdisciplinary treatment team, ensuring consistent, clear, and timely communication
- Coordinate and monitor the implementation of services and supports outlined in each student’s IEP and Individualized Plan of Protective Oversight (IPOP) under the direction of the Family Liaison
- Facilitate and document treatment team meetings, admission meetings, transition meetings, and move meetings, ensuring timely distribution of minutes and follow‑up
- Develop, maintain, distribute, and quarterly review IPOP documentation, ensuring implementation across educational and residential settings
- Provide and document routine family contact, supporting parents/guardians with systems navigation related to insurance, SSI, Medicaid, guardianship, and related processes
- Collaborate with Admissions, program teams, and external agencies to support successful student admission, transition, aging‑out planning, adult placement, and discharge
- Assist with intake activities, placement meetings, screenings, and coordination of required discharge and transition documentation
- Maintain accurate, up‑to‑date student records in Therap in accordance with regulatory and agency requirements
- Participate actively as a member of the Interdisciplinary Treatment Team, promoting collaboration, accountability, and timely completion of documentation
- Identify and address record discrepancies to ensure compliance and data integrity
- Support Development Department initiatives, including fundraising, marketing, and events as appropriate
- Maintain mandated trainings, engage in ongoing professional development, and participate in crisis intervention and emergency response as required
REQUIREMENTS:
- Bachelor’s degree in Human Services or related field preferred, with sufficient experience to perform job responsibilities
- Experience working with children, adolescents, and families in human services or special education settings preferred
- Ability to use Microsoft 365 and electronic documentation systems proficiently
- Flexible schedule required; evenings and weekends may be necessary
- Must maintain agency‑required crisis intervention certification
- Ability to assist with physical and emergency response requirements, including evacuation procedures
